GENIE Test for CIRS: Ask What the Result Would Change

The short answer

A reader put the access question plainly: "Can you get a Genie through mold co?" No. MoldCo doesn't offer, order, perform, or interpret GENIE. None of MoldCo's current panels is a transcriptomic substitute for it.
The access question has a direct answer. Before pursuing GENIE elsewhere, define the harder question: the decision you need the result to change. That could mean whether it supports a diagnosis, a claim about the source of an exposure, a treatment choice, or what to investigate next. The published evidence doesn't support treating GENIE alone as a definitive CIRS diagnosis. It also doesn't establish that the test can prove one person's exposure source or choose treatment automatically. The test may still supply information. Its value depends on whether that information is solid enough for the job you give it.
Molecular precision and decision-grade proof aren't the same thing.
What GENIE is measuring
Your DNA carries the genetic instructions you inherited from your parents. Cells use those instructions to make RNA and other functional products. Gene expression describes that activity when a sample is taken. It can differ by cell type and change under different conditions, which is why the National Human Genome Research Institute describes it as a regulated process rather than a fixed genetic verdict.
That distinction matters. An inherited-DNA or HLA test asks about variants that are generally stable. A gene-expression test looks at activity captured at a particular time. A detailed activity pattern can be biologically interesting without telling us, by itself, which condition caused it or which clinical action should follow.
The report's complexity can make it feel conclusive. It isn't a substitute for validation.
Put the research back in its real population
The foundational human study was small and specific. It compared 11 selected patients with chronic illness after ciguatera poisoning with 11 age-matched healthy controls. Researchers ran the blood samples in technical duplicate, meaning they repeated the measurement. They didn't study 44 people.
In the study's statistical analyses, the expression profiles of the 11 selected ciguatera-associated cases separated from those of the 11 controls. For seven genes, the microarray and PCR measurements showed a reported 95% correlation. That was a signal worth studying. It wasn't a prospective test of routine commercial accuracy in a representative group of people being evaluated after exposure to water-damaged buildings. It didn't establish sensitivity, specificity against competing diagnoses, individual causation, or improved outcomes after treatment.
Later work didn't close those gaps. A 2021 observational study from researchers who developed the framework reported that some environmental Actinobacteria sequencing measurements moved with selected gene-activation patterns. Movement together doesn't prove which exposure caused one person's result. That takes a stronger design and independent confirmation.
Even authors writing a 2025 narrative review favorable to the framework acknowledged that no single large-scale GENIE validation study had been published. This wasn't an independent systematic validation, so it shouldn't carry more weight than its design allows. But the admission is useful: there is still a distance between a promising molecular framework and a test validated for every claim a reader may hope to make from the report.
Three different jobs often get blurred together
Three questions separate an impressive measurement from a useful clinical test. The FDA's general education on medical tests distinguishes these layers. It's a general framework, not an FDA determination about GENIE.
First comes analytical validity, the laboratory's ability to measure the intended biological signal accurately and reliably. For GENIE, it means examining the RNA measurement itself, its repeatability, and the rules used to turn measurements into a report. A reliable measurement matters, but it doesn't yet tell you whether the result answers your clinical question.
Next comes clinical validity. The pattern must reliably identify the claimed condition in relevant patients, including people with other possible explanations for the same symptoms. A result that separated two small, selected ciguatera-associated groups doesn't automatically perform the same way among people with overlapping symptoms and other possible diagnoses. Until that standard is met for the intended use, a precise report can't settle the diagnosis by itself.
Finally, clinical utility turns to care. The result must change what the clinician does, and that change must produce a better outcome than the available alternatives. A report can perform well at the measurement layer while clinical validity and clinical utility remain unsettled.
The sequence matters. If a test adds detail without resolving the decision that led you to test, the extra findings can increase the interpretation burden without increasing the report's value.
Take a decision plan to the clinician
GENIE should be considered with a qualified clinician who can explain both the report and the limits of the evidence. Before ordering, write down the decision you expect the result to change. Then ask:
- What would we do after each possible result, including an unclear or conflicting result?
- Which interpretations have been validated independently, in patients like me, rather than inferred from a small selected cohort?
- Which pre-analytic factors matter, and what evidence supports the instructions?
- Who will interpret the report, and how will uncertain findings be kept from turning into a diagnosis or causal claim they can't support?
- Is there another, better-validated step that answers my immediate clinical question with less interpretation burden?
These questions aren't a reason to dismiss molecular testing. They keep an interesting measurement from being mistaken for an answer it hasn't earned.
If you can't name the decision that changes after the result, settle that with your clinician before pursuing the test. A complicated report shouldn't be the place where the purpose of testing is discovered.
